PPAZ is a volunteer-led organisation with 33 branches in 10 provinces overseen by four regions and an elected national executive committee (NEC). The PPAZ head office, based in Lusaka, provides oversight and supervises the overall implementation of programmes and projects.

PPAZ operates four reproductive health centres (RHCs) in Kitwe, Lusaka, Choma, and Livingstone, which are open to the public from Monday to Saturday, except Sundays and public holidays. Through these RHCs, PPAZ delivers services through four service delivery channels: static, associated public and private health facilities, mobile (outreach), and community-based distributors. PPAZ also has a project office in Ndola, which implements community-operated projects in partnership with the Japanese Ministry of Foreign Affairs and the Japanese Organisation for International Cooperation in Family Planning (JOICFP). Through these service delivery channels, projects, and volunteer structures, PPAZ has established its presence in selected districts in all provinces in Zambia.
